EPI-PEN AUTO-INJECTOR FORM
A Medication Consent Form must be completed each time your child is to receive any doctor-prescribed medicine at school. A new form needs to be filled out every year. A physician’s order form is not needed if your child is to receive over-the-counter medicine. However, we need the parent/guardian consent form completed and sent to school with the medicine in the original container. If your child will keep an albuterol inhaler at school, an Asthma Inhaler Administration Authorization Form must be completed by parent and physician.
